Gracepoint offers a Sliding Fee Discount Program to all patients of the health center based on income and household size. We will care for you regardless of your ability to pay.  If interested in this option, more details will be provided when you visit.

We are open Monday - Friday 8-5 pm  Walk-in appointments are welcome.

What do I bring to my first appointment?

 If you are a new patient, please bring:
1. Your photo ID
2. Insurance card
3. Money for co-pay
4. Bring all medications
5. Please visit the Resources link at the right of this page to complete the New Patient Information Packets

If you do not currently have insurance, 

You will need to provide proof of income for everyone in your household.  We recommend that you bring your prior year tax return as proof of income.   If you do not have a tax return, you may bring the following items:

•   Most recent pay stubs for one month
•   Unemployment verification or check stubs
•   Social Security payment statement
•   Social Security disability  statement or check stubs
•   Retirement payments
•   Alimony payments
All information cannot be older than 60 days

Genoa Pharmacy

Genoa Healthcare, located on Gracepoint's campus is the largest provider of pharmacy, outpatient telepsychiatry and medication management services serving people requiring behavioral health and addiction treatment. They aim to improve the lives of individuals living with a mental disorder and/or battling addiction. Striving to be the leading provider of specialized services catering to those who suffer from chronic illnesses and their families.

Genoa Healthcare Pharmacy is available for walk-in prescriptions, provides door to door delivery services and convenient pick up options at no additional cost. Personalized prescription packages to patient medication needs may also be available. Patients are able to track medication using an online account through Genoa with the goal to improve consumer’s lives by making medications accessible.
